This module looks at the role of community pharmacy in identifying people who may have high blood pressure.

This module looks at the role of community pharmacy in identifying people who may have high blood pressure — a new service in England’s community pharmacy contractual framework. 

It describes the role of ambulatory and home blood pressure monitoring and explains the current definitions of blood pressure levels that are normal, high or very high, and any action that community pharmacy teams should take.

The module goes through the information that community pharmacists should transfer to GP practices having identified a patient with high blood pressure.
